Directions
Elite Physical Therapy in Forest is located at 339 West Third Street. If you are traveling from I20 you will take a R at the Intersection of Hwy 80 and 35. If you are coming South on Hwy 35 you will take a L. Head East on Hwy toward Lake. Elite is located in the shopping center directly behind AutoZone and across the Street from Tractor Supply.
